Fluid Accumulation in the Middle Ear in Children

Serous otitis media is most common in children between 6 months and 6 years of age. Since the Eustachian tube is not yet fully developed in this age group, the ventilation of the middle ear can be easily impaired. Starting preschool, frequent upper respiratory tract infections, and enlarged adenoids are factors that increase the risk.

The main causes that can lead to fluid accumulation in the middle ear are:
• Frequent upper respiratory tract infections
• Allergic rhinitis
• Enlarged adenoids (adenoid hypertrophy)
• Eustachian tube dysfunction
• Exposure to cigarette smoke
• Genetic predisposition
These factors can disrupt the ventilation of the middle ear, causing fluid accumulation

Serous otitis media is often unnoticed by families because it is painless. The most common symptoms are:
• Mild or moderate hearing loss
• Feeling of fullness or blockage in the ear
• Sometimes stinging pain
• Balance problems
• Delay in speech and language development
• Decline in school performance
Especially in long-term cases, hearing loss can negatively affect the child’s social and academic development.

The diagnosis is made by ear, nose, and throat examination. Dullness, collapse, or fluid level can be observed in the eardrum examination. In necessary cases:
• Tympanometry
• Audiometry (hearing test)
Middle ear pressure and hearing status are evaluated.

A large part of serous otitis media cases can resolve spontaneously within the first 3 months. For this reason, regular follow-up is preferred in most patients in the early period. However, treatment may be required if the fluid persists for a long time or if significant hearing loss occurs.

Treatment is planned according to the duration of the disease and the severity of the symptoms.
1. Observation and Follow-up
The course of the disease is monitored with regular check-ups in mild cases.
2. Medical Treatment
• Nasal sprays
• Allergy treatments
• Drug treatment when necessary
These treatments aim to improve the function of the Eustachian tube.
3. Surgical Treatment
• Insertion of ventilation tube (ear tube)
• Adenoidectomy if there is adenoid enlargement
Surgical treatment is generally recommended in cases that last longer than 3 months, lead to hearing loss, or recur frequently.

Permanent hearing loss may develop in cases of serous otitis media that are not treated and persist for a long time. This situation poses a risk, especially in terms of speech and language development.

Although it cannot be completely prevented, the risk can be reduced:
• Keeping the child away from cigarette smoke
• Regular allergy treatment
• Treatment of nasal congestion
• Paying attention to protection from infections
