Temporomandibular Joint (TMJ) Disorders

What is Temporomandibular Joint (TMJ)?
The temporomandibular joint (TMJ) is a complex joint that connects the lower jaw to the skull and enables the healthy performance of daily functions such as speaking, chewing, and swallowing. This joint consists of bone, cartilage, ligaments, and muscles and is one of the most active joints in the body.
Temporomandibular joint disorders arise from functional or structural problems affecting these structures and can significantly reduce the quality of life.

Frequently Asked Questions About Temporomandibular Joint (TMJ) Disorders

Temporomandibular joint disorders encompass a group of diseases characterized by pain in the temporomandibular joint and the surrounding muscles, limited movement, sound, and loss of function. It can occur alone or may manifest with complaints that spread to the ear, head, and neck region.

Temporomandibular joint disorders usually occur due to a combination of factors:
• Trauma: Blows to the face or jaw area
• Bruxism (Teeth grinding and clenching): Especially stress-related
• Posture disorders: Tension in the neck and shoulder muscles
• Malocclusion: Unbalanced bite and jaw position
• Stress and anxiety: Can lead to continuous muscle contraction

Common symptoms that may be seen in TMJ disorders include:
• Restriction or locking in jaw movements
• Clicking or crackling sound when opening or closing the mouth
• Pain in the jaw, face, ear, and temple area
• Headaches and migraine-like complaints
• Feeling of fullness or tinnitus in the ear
• Sensitivity in the teeth and imbalance in occlusion
These symptoms can sometimes be confused with ear diseases, and patients may apply to ENT clinics.

Diagnosis begins with a detailed patient history and physical examination. When necessary:
• Panoramic dental X-ray
• Magnetic resonance imaging (MRI)
• Computed tomography (CT)
The bone and soft tissue structures of the joint are evaluated.

Treatment is planned according to the underlying cause and the severity of the complaints.
1. Conservative (Non-Surgical) Treatments
• Applying hot or cold compresses
• Eating soft foods
• Avoiding excessive mouth opening
• Stress management and relaxation techniques
2. Physical Therapy and Exercises
Exercises to relax the jaw muscles and increase joint range of motion can be applied.
3. Intraoral Appliances (Splints)
Night splints can alleviate the load on the joint by reducing teeth clenching and grinding.

4. Medical and Interventional Treatments
• Pain relievers and muscle relaxants
• Botulinum toxin (Botox) applications
• Surgical intervention in rare cases

• Avoiding dusty and pollen-filled environments
• Not keeping pets at home
• Not using long-pile carpets and heavy curtains
• Avoiding perfumes, room fresheners, and irritating chemicals
• Developing a regular nasal washing habit
